This can be completed physically and returned to The Good Life Institute by email, fax, mail, or dropped of at the Town Office, as indicated in the contract.
This can be done online by completing the following information and pressing send, indicating that you have read the terms of the contract:

THE SPECIAL EVENT VENDOR NOTIFICATION IS IMPERATIVE TO PARTICIPATION AND MUST BE COMPLETED AS SOON AS POSSIBLE FOR OUR LOCAL HEALTH INSPECTOR.
